Okazało się, że miesięczne dawki 60 000 IU witaminy D nie zmniejszały ogólnego ryzyka ostrej infekcji dróg oddechowych, ale mogły nieznacznie skrócić czas trwania objawów.

    Amantadyna to lek, który został opracowany jako inhibitor wirusa grypy. Jest to substancja, która hamuje aktywność białka M2 wirusa grypy (białko to wykazuje aktywność kanału jonowego i uczestniczy w inicjacji procesu zakażenia).

    Chociaż jest to ciekawa koncepcja, nikt do tej pory nie pokazał, że – nawet w badaniach in vitro – amantadyna hamuje replikację wirusa poprzez interferencje z aktywnością kanału jonowego. Nie ma więc w tej chwili żadnych dowodów na to, że amantadyna może hamować zakażenie SARS-CoV-2. Mówię o dowodach naukowych.

    Wyniki tego badania wskazują na nieskuteczność amantadyny, a sami autorzy zwracają uwagę na brak zasadności stosowania tego leku u chorych na COVID-19 do czasu uzyskania dowodów większej jakości pochodzących z badań klinicznych z randomizacją.

    After 30 days, researchers found no significant differences in patients' symptoms or health. The mortality rate was nearly the same at 11% in the convalescent plasma group and 11.4% in the placebo group, a difference not deemed statistically significant.

    Dane te pokazują, że remdesivir był lepszy od placebo pod względem skrócenia czasu powrotu do zdrowia u dorosłych, którzy byli hospitalizowani z powodu Covid-19. Dane z tego badania sugerują również, że leczenie remdesiwirem mogło zapobiec progresji do ciężkiego przebiegu choroby. Wskazuje na to rzadsze występowanie niewydolności oddechowej wśród pacjentów z grupy leczonej remdesivirem, a także niższa częstość stosowania u nich tlenoterapii.

  • New AI diagnostic can predict COVID-19 without testing

    In this study, the researchers analysed data gathered from just under 2.5 million people in the UK and US who had been regularly logging their health status in the app, around a third of whom had logged symptoms associated with COVID-19. Of these, 18,374 reported having had a test for coronavirus, with 7,178 people testing positive.

    The research team investigated which symptoms known to be associated with COVID-19 were most likely to be associated with a positive test. They found a wide range of symptoms compared to cold and flu, and warn against focusing only on fever and cough. Indeed, they found loss of taste and smell (anosmia) was particularly striking, with two thirds of users testing positive for coronavirus infection reporting this symptom compared with just over a fifth of the participants who tested negative. The findings suggest that anosmia is a stronger predictor of COVID-19 than fever, supporting anecdotal reports of loss of smell and taste as a common symptom of the disease.

    The Surviving Sepsis Campaign panel recently recommended that “mechanically ventilated patients with COVID-19 should be managed similarly to other patients with acute respiratory failure in the ICU [1].”
    Yet, COVID-19 pneumonia [2], despite falling in most of the circumstances under the Berlin definition of ARDS [3], is a specific disease, whose distinctive features are severe hypoxemia often associated with near normal respiratory system compliance (more than 50% of the 150 patients measured by the authors and further confirmed by several colleagues in Northern Italy). This remarkable combination is almost never seen in severe ARDS. These severely hypoxemic patients despite sharing a single etiology (SARS-CoV-2) may present quite differently from one another: normally breathing (“silent” hypoxemia) or remarkably dyspneic; quite responsive to nitric oxide or not; deeply hypocapnic or normo/hypercapnic; and either responsive to prone position or not. Therefore, the same disease actually presents itself with impressive non-uniformity.
  • Evidence That Vitamin D Supplementation Could Reduce Risk of Influenza and COVID-19 Infections and Deaths

    The world is in the grip of the COVID-19 pandemic. Public health measures that can reduce the risk of infection and death in addition to quarantines are desperately needed. This article reviews the roles of vitamin D in reducing the risk of respiratory tract infections, knowledge about the epidemiology of influenza and COVID-19, and how vitamin D supplementation might be a useful measure to reduce risk. Through several mechanisms, vitamin D can reduce risk of infections.
    To reduce the risk of infection, it is recommended that people at risk of influenza and/or COVID-19 consider taking 10,000 IU/d of vitamin D3 for a few weeks to rapidly raise 25(OH)D concentrations, followed by 5000 IU/d. The goal should be to raise 25(OH)D concentrations above 40-60 ng/mL (100-150 nmol/L). For treatment of people who become infected with COVID-19, higher vitamin D3 doses might be useful. Randomized controlled trials and large population studies should be conducted to evaluate these recommendations.

    W wydanym 12 kwietnia br. komunikacie WHO podkreśla, że aktualnie nie ma dowodów naukowych potwierdzających, że szczepionka BCG zapobiega zakażeniu SARS-CoV-2. Z przeprowadzonego przez WHO przeglądu dostępnych danych wynika, że do 11 kwietnia br. dostępne były trzy nierecenzowane publikacje wstępnych obserwacji, w których autorzy porównali zapadalność i umieralność z powodu COVID-19 w krajach, w które prowadzą programy powszechnych szczepień BCG i które nie prowadzą takich programów. Choć wyniki tych analiz potwierdziły założone przez autorów hipotezy, to należy podkreślić, że tego typu badania, opierające się na prostej analizie związku (korelacji) zmiennych, obciążone są wieloma błędami systematycznymi i w żadnym wypadku nie świadczą o istnieniu związku przyczynowo-skutkowego.

    Osocze, które może być wykorzystywane do leczenia, jest wytwarzane z krwi pobranej od osoby z przebytym zakażeniem koronawirusem. W terapii wykorzystane są przeciwciała, które wytworzył układ immunologiczny tej osoby w odpowiedzi na zakażenie.

    „Liczymy na to, że przeciwciała, które mają ozdrowieńcy, mają właściwości neutralizujące, a więc są w stanie unieruchomić, zniszczyć tego wirusa. Przetoczenie takiego preparatu ma na celu właśnie pozbawienie pacjenta wirusów, które krążą w jego ustroju, może nie wszystkich, ale dominującej części” – wyjaśnił prof. Tomasiewicz.

    Osocze powinno być podawane na przełomie pierwszej i drugiej fazy choroby, to znaczy „w okresie narastania objawów, ale jeszcze przed wystąpieniem niewydolności oddechowej”. Jak wyjaśnił, osoba z niewydolnością oddechową cierpi z powodu tzw. burzy cytokinowej, „czegoś, co już się dokonało” w organizmie, i podanie osocza dopiero na takim etapie choroby może nie spowodować poprawy stanu zdrowia.

    Mortality could be as high as two thirds among patients with COVID-19 who require ventilation, new data from the United Kingdom's Intensive Care National Audit and Research Center (ICNARC) show.
    Some clinicians are asking whether other techniques, such as extracorporeal membrane oxygenation (ECMO), could improve outcomes, but the data are unclear.

    The ICNARC data, posted online April 10, include data from 3883 patients with confirmed COVID-19 who were admitted to intensive care units (ICUs) in England, Wales, or Northern Ireland and for whom data on the first 24 hours of ICU care are available.
    Of those, 871 patients died, 818 patients survived to ICU discharge, and 2194 patients were still receiving ICU care.
    Among patients whose ICU outcome is known, 66.3% of the 1053 patients who required mechanical ventilated died, compared with 19.4% of the 444 patients who required basic respiratory support.

    Importantly, mortality among patients with COVID-19 who require mechanical ventilation appears higher than that for patients with other types of viral pneumonia. Specifically, the ICNARC report shows a mortality rate of 35.1% among patients who were treated in the ICU for viral pneumonia and who required mechanical ventilation from 2017 to 2019. The new ICNARC findings are consistent with previous reports of smaller case series. For example, a single-center case series of 52 patients treated in Wuhan, China, showed that 37 (71%) required mechanical ventilation and 32 (61.5%) died within 28 days of ICU admission. Morality was higher among those who required mechanical ventilation than among those who did not (94% vs 35%).

    it is incumbent on researchers to accelerate clinical trials of any readily available and potentially acceptably safe therapies that could reduce the rising death toll. Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) gains access to host cells via angiotensin-converting enzyme 2, which is expressed in the type II surfactant-secreting alveolar cells of the lungs.

    Severe COVID-19 is associated with a major immune inflammatory response with abundant neutrophils, lymphocytes, macrophages, and immune mediators. Which mediators are most important in driving the immune pathology remains to be elucidated. Deaths from COVID-19 are chiefly due to diffuse alveolar damage with pulmonary oedema, hyaline membrane formation, and interstitial mononuclear inflammatory infiltrate compatible with early-phase adult respiratory distress syndrome (ARDS).

    Prevention of ARDS and death in patients with COVID-19 is a pressing health emergency.

    Anti-tumour necrosis factor (TNF) antibodies have been used for more than 20 years in severe cases of autoimmune inflammatory disease such as rheumatoid arthritis, inflammatory bowel disease, or ankylosing spondylitis. There are ten (as reported on Sept 29, 2019) US Food and Drug Administration approved and four off-label indications for anti-TNF therapy,indicating that TNF is a valid target in many inflammatory diseases. TNF is present in blood and disease tissues of patients with COVID-19 and TNF is important in nearly all acute inflammatory reactions, acting as an amplifier of inflammation. We propose that anti-TNF therapy should be evaluated in patients with COVID-19 on hospital admission to prevent progression to needing intensive care support.

    It is difficult to find a product that currently has a better established safety profile than chloroquine. Furthermore, its cost is negligible. Hence, its possible use both in prophylaxis in people exposed to the novel coronavirus and as a curative treatment will probably be promptly evaluated by our Chinese colleagues. If clinical data confirm the biological results, the novel coronavirus-associated disease will have become one of the simplest and cheapest to treat and prevent among infectious respiratory diseases.

    Our findings reveal that remdesivir and chloroquine are highly effective in the control of 2019-nCoV infection in vitro. Since these compounds have been used in human patients with a safety track record and shown to be effective against various ailments, we suggest that they should be assessed in human patients suffering from the novel coronavirus disease.
